RETURN-TO-DUTY

PROCOM supports Return-to-Duty testing programs in accordance with Department of Transportation requirements. The Return-to-Duty process is highly regulated, and eligibility must be confirmed before testing can proceed.

This form collects only preliminary information and is used to assess eligibility against the required criteria. Submission does not guarantee approval or advancement in the Return-to-Duty process.

Important eligibility notice:
If you are not an owner-operator with your own DOT number, or you are not employed by a company that will manage your Return-to-Duty and follow-up testing, American Drug Testing Consortium cannot administer your Return-to-Duty process under FMCSA 49 CFR Part 40 Subpart O.
